Approche à la prise en charge de l’arthrite associée aux maladies inflammatoires de l'intestin

La manifestation extraintestinale la plus courante de la maladie intestinale inflammatoire (MII) est l’arthropathie. Ces pathologies ont été signalées chez près de 50 % des patients atteints de MII et sont plus fréquentes dans le cas de la maladie de Crohn (MC), en particulier de la maladie colique, et chez les femmes.
L'arthrite associée aux MII est classée comme un type de spondylarthrite (SpA). Le traitement dépend du type d’atteinte de la SpA, qui peut être subdivisée en atteinte périphérique ou axiale. L’approche thérapeutique consiste en une combinaison de thérapies non pharmacologiques et pharmacologiques gérées par une équipe multidisciplinaire et repose sur des décisions concertées entre les services de gastro-entérologie et de rhumatologie. Compte tenu de l’expansion rapide de l’arsenal thérapeutique pour l’arthrite assocée aux MII et l'arthrite à mediation immunitaire, cet article présente une approche du traitement de l'arthrite associée aux MII, en tenant compte des recommandations formulées dans des lignes directrices récentes et des nouveaux traitements.
